Group Life Disability Claim Process

You will find below all details on how to submit your claim and the ensuing processes in the claim management. There will be no complications and we will assure a speedy claim settlement.

CLAIM INTIMATION

Send an email to life.claims@sukoon.com within 30 calendar days from the date of the accident/diagnosis with the following details:

Group life policy number
Employee name and contact number
Date of joining
Date of birth
Date, place and details of accident/sickness
Basic monthly salary at the time of the event

We will send you your claim reference number within 3 working days of receiving your intimation email.

DOCUMENTS REQUIRED

Send the following documents to life.claims@sukoon.com along with a completed, stamped and signed claim form, within 30 days of receiving the claim reference number:

 

  • Duly filled disability claim form
  • Claim reference number
  • Medical report detailing the diagnosis and treatment
  • Doctor’s prescription for the medicines purchased
  • X-ray and laboratory reports
  • Salary slips for the three months prior to the date of the event, comprising a breakup of the basic salary and fixed allowances
  • A copy of the passport and the visa page 
  • Disability certificate provided by the Government Medical Board in the UAE (in cases of permanent total and partial disability)
  • Original sick leave certificate for the period of absence from work (in cases of temporary total disability)
  • Original medical expenses invoices (in cases of accidental medical expenses)

We may ask for additional documents as and when they are required for the claim processing.

CLAIM PROCESSING

You can expect to receive the applicable claim settlement and/or our response within 14 working days of submitting the required documents.

CLAIM SETTLEMENT

Once the claim is approved, a discharge receipt will be sent to you, detailing the amount you will be eligible to receive within 7 working days of claim intimation. After we receive all documents, the amount will be transferred to their bank account within 14 working days.
